I, am aware that unity academy, a division of Unity Sports and Entertainment, its affiliates, sponsors, staff, volunteers and supporters are not responsible for any injury, death, or damage to personal property before, during or after each game played. All players and spectators assume all risk by attending, participating and waive all liability and claims against unity sports and entertainment.
Unity Academy photographs, and records audio of all games. Your presence will be deemed your consent to the unconditional use of your image, voice and likeness in all media forever for the purpose of advertising, marketing, publicity and promotion of the league.
I, confirm that I have read and understood each term in this waiver and have freely and voluntarily agreed to these terms.
I will follow all league rules and policies. I understand that failure to do so will result in suspension or life time ban from league.
